The facility shall take all necessary precautions to assure the safety of the residents at all times.
The 1981 Edition of the National Fire Protection Association (NFPA) Standard No. 101, Life Safety Code for existing structures and all appropriate references under Appendix "B" of that Code but no subsequently amended edition of the Code, shall apply to and became a part of these standards.
Based on observation and staff interview the facility failed to maintain one required exit door in operable condition. Refer to: 1967 E LSC K038

Exit access is so arranged so that exits are readily accessible at all times.
Findings include:
During environmental tour on 7/20/99 at 3:10 p.m. surveyor, accompanied by E4, observed that the designated fire exit door at the end of the 200 wing was taped shut with duct and masking tape. Tape covered the door/jamb junctures on the lower half of the door to the height of the panic hardware.
E4 attempted to open door by placing his full force on the panic hardware and the door, but the door would not open. E4 then used a utility knife to slit the tape and the door was freed.
During interview E4 stated that the door had been taped "since winter" in effort to stop cold air drafts.
E5 stated during interview that E4 had taped the door to resolve a resident family concern about cold drafts.
