Hammond House Facility I.d. Number: 0030619 Complaint Investigation "A" VIOLATION(S): AN OWNER, LICENSEE, ADMINISTRATOR, EMPLOYEE OR AGENT OF A FACILITY SHALL NOT ABUSE OR NEGLECT A RESIDENT. A FACILITY EMPLOYEE OR AGENT WHO BECOMES AWARE OF ABUSE OR NEGLECT OF A RESIDENT SHALL IMMEDIATELY REPORT THE MATTER TO THE FACILITY ADMINISTRATOR. A FACILITY ADMINISTRATOR WHO BECOMES AWARE OF ABUSE OR NEGLECT OF A RESIDENT SHALL IMMEDIATELY REPORT THE MATTER BY TELEPHONE AND IN WRITING TO THE RESIDENTS REPRESENTATIVE. These regulations are not met based on the following: Per an Incident Report dated 4/7/03, at 7:00 p.m., during shower time, staff noticed several bruises on R1. Staff noticed one bruise in the middle of R1's left arm and 3 bruises on the upper inner left arm. Staff asked R1 what happened and she stated E7 hit her several times. Staff informed E1, RSD, QMRP, (Residential Services Director, Qualified Mental Retardation Professional) of the allegation of abuse to R1 by E7. Per the Investigation of the allegation of abuse to R1 by E7 dated 4/8/03, E4, Program Aide, worked with E7 on 4/5/03 and 4/6/03. E4 told E1, (RSD, QMRP) that on the morning of 4/5/03 she and E7 were getting the residents up. E7 went to get R1 up and she wouldn't cooperate. E7 tried to get R1 up a couple of times but she refused. At 1 point R1 cursed at E7. E4 said that by that time E7 was in a rage and she followed E7 because she felt something was going to happen. E4 stated that she saw E7 hitting R1. E4 said E7 was standing in front of R1 and R1 was down on the floor. E4 stated she immediately turned around and went and carried out her shift duties. E1 asked E4 did she try to intervene and she said, "No". E1 asked E4 why she didn't page her ( E1) and call the police. E4 said she really didn't have a reason why she didn't inform her supervisor or call the police. E5, Program Aide, was interviewed by E1 on 4/7/03. E5 stated that on 4/6/03, at approximately 6:45 p.m., during shower time, that she saw 4 bruises on R1's left arm. E5asked R1 what happened and she (R1) stated that E7 hit her. R1 also stated that E7 tore the arm off her teddy bear. E5 stated that she didn't know what to do, so she showed the bruises to her co-worker, E4. E4 stated that R1 had blood drawn a few days ago. E5 stated that she didn't do anymore inquiring. E8, Director of Habilitation Services, was interviewed on 4/17/03 at 10:40 a.m. E8 was asked if the facility notified IDPH (Illinois Department Of Public Health) of the abuse allegation involving R1. E8 stated after Z1 had visited the facility and been informed of the allegation of abuse she had asked the facility to notify IDPH. E8 produced notification of IDPH dated 4/15/03 which is 10 days after the abuse allegedly took place and 8 days after the facility was made aware of the abuse allegation. E8 was asked why IDPH was not immediately notified. E8 stated we did not notify IDPH because we didn't feel it was serious enough. We made sure she was in a safe environment. I, (E8) explained this to the Guardian and she asked us to notify IDPH which we did. |
