The Illinois Department of Public Health (IDPH) is proud to sponsor various activities to commemorate 2012 Emergency Medical Services (EMS) Week.

EMS Week Certificates

IDPH will provide certificates for years of service in EMS in five year increments (five, ten, fifteen, twenty, twenty-five years, etc.) for all levels of pre-hospital personnel (Basic, Intermediate, Paramedic, First Responder, First Responder Defibrillator, Lead Instructor, Emergency Medical Dispatcher, Emergency Communications RN, Pre-Hospital RN and Trauma Nurse Specialist).

The deadline for submission of names to the Department for certificates is April 20, 2012. Please ensure that all names are spelled correctly and that the correct level and the number of years of EMS service are indicated for each entry.

Forms will only be accepted in electronic format on the prescribed form. No other means of submission will be accepted. Please pay close attention to the instructions when filling out the form. Please e-mail completed forms to Paula Atteberry at Paula.Atteberry@illinois.gov . Certificates will be mailed to the EMS System Coordinator for distribution.

EMS Week Awards

Awards for EMS Instructor, Public Education, Response/Professional and Response/Layperson will be given again this year. Nominations for an “EMS Week Award" must be submitted to the Department by April 6, 2012. Please e-mail completed forms to Paula Atteberry at Paula.Atteberry@illinois.gov .
