PEORIA NURSING HOME FINED

SPRINGFIELD, IL – The Illinois Department of Public Health has fined Rosewood Care Center of Peoria $10,000 (Statement of Violations) for failure to provide adequate and proper nursing care to prevent a resident death. The 120-bed skilled care facility is located at 1500 W. Northmoor Road in Peoria.

Responding to a complaint, Department investigators learned that a 77-year-old resident suffered a head injury in a fall at the facility, but staff did not consistently monitor the resident's neurological condition and failed to notify the man's physician in a timely manner when his condition declined. More than eight hours after the injury and five hours after his condition began to deteriorate, the resident was transported to a local hospital emergency department where he died from a brain hemorrhage.

A Department-imposed plan of correction ordered the facility to implement procedures to ensure a resident's physician is notified of any accident, injury or significant change in condition that threatens his/her health, safety or welfare.

Rosewood Care Center of Peoria has requested a hearing on the Department's action. A hearing date has not been scheduled.
