ROSEWOOD CARE CENTER FINED FOR INADEQUATE SUPERVISION AND PATIENT CARE
SPRINGFIELD, IL -- Rosewood Care Center of Peoria has been fined $10,000 (Statement of Violation) by the Illinois Department of Public Health for failure to provide adequate supervision to prevent a resident from wandering from the facility. The 120-bed skilled care facility is located at 1500 W. Northmoor Road in Peoria.
Responding to a complaint, Department surveyors learned that an 81-year-old resident left the care center unnoticed on the morning of Jan. 25, apparently through an employee entrance. The woman, who was wearing only pajamas and slippers, was discovered lying in a pool of blood on the facilitys concrete driveway by a staff member arriving for work.
The resident was transported to a local hospital where she was treated for a nasal fracture, facial abrasions and cuts. Following her return to the facility the same day, the residents condition was not closely monitored by staff as required by facility policy when there is a head injury. Less than three hours later, the womans condition deteriorated and she was again transported to the hospital where she was admitted and remained for six days.
Surveyors determined through staff interviews that an alarm on the employee entrance was manually turned off and not operational when the resident walked out.
Rosewood Care Center has requested a hearing on the Departments action. A hearing date has not been scheduled.
