SOUTH BELOIT NURSING HOME FINED

SPRINGFIELD, IL – The Illinois Department of Public Health has fined Fair Oaks Health Care Center $5,000 (Statement of Violation) for failure to provide adequate supervision to prevent a resident elopement. The 65-bed skilled care facility is located at 1515 Blackhawk in South Beloit.

Responding to a complaint, Department surveyors learned that an 89-year-old resident left the facility unnoticed and was observed by a regular facility visitor at a nearby flower shop. In order to reach the flower shop, the resident had to cross a four-lane highway. She is believed to have left through an exit door whose alarm system was broken. The resident has impaired judgement skills and must be supervised continually. She was not injured.

The Department ordered a plan of correction that included maintaining exterior door alarms and ensuring constant visual supervision for all resident-accessible exterior doors not equipped with alarms.

Fair Oaks Health Care Center has requested a hearing on the Department’s action. A hearing date has not been scheduled.
