BLOOMINGDALE NURSING HOME DISCIPLINED

SPRINGFIELD, IL – The Illinois Department of Public Health has fined Bloomingdale Pavilion $5,000 (Statement of Violation),for failing to take the necessary precautions to prevent a resident from leaving the facility unsupervised. The 259-bed skilled care facility is located at 311 Edgewater Drive in Bloomingdale.

As part of an annual survey, Department surveyors learned a resident, who had a history of wandering and wore an electronic monitor, was able to leave the facility unnoticed when his monitor failed to activate an exit door alarm. He was found by police at a gas station about two miles away and returned to the facility unharmed.

According to the resident's care plan, facility staff were supposed to conduct hourly visual checks monitoring the resident's whereabouts and to check his electronic bracelet every shift to ensure it was functioning properly. There was no documentation to support his care plan was followed.

A Department-ordered plan of correction required the facility to properly supervise its residents and to ensure all resident-accessible exterior doors are equipped with functional alarms or are monitored by constant visual supervision.

Bloomingdale Pavilion has requested a hearing on the Department's actions. No hearing date has been scheduled.
