GODFREY FACILITY FINED FOLLOWING RESIDENT'S DEATH

SPRINGFIELD, IL – The Illinois Department of Public Health has fined D'Adrian Convalescent Center $10,000 (Statement of Violation) for failure to properly assess a resident's medical needs and to notify her doctor when her condition deteriorated. The 119-bed skilled and intermediate care facility is located a 1373 D'Adrian Professional Park in Godfrey.

Responding to a complaint, Department surveyors learned a resident was hospitalized after a urinary tract infection was left untreated and the woman developed a severe blood infection. She died a few hours after being admitted to the hospital.

According to facility records, the resident was being treated for vaginitis when she first complained of feeling ill. By the next day, she was weak, unable to get out of bed and refused to eat.

The resident's physician ordered laboratory tests after an examination, but the facility did not notify the doctor when the results were received. A blood test indicated an infection, which her doctor said should have been treated with an antibiotic.

The physician was not notified of the resident's deteriorating condition until three days later when the resident was hospitalized.

The Department-ordered plan of correction required facility administration to ensure its staff conducts comprehensive assessments of residents and, when a significant change of condition is observed, to ensure proper physician notification.

D'Adrian Convalescent Center has not requested a hearing on the Department's action.
