WHEELING FACILITY FINED FOR INADEQUATE SUPERVISION

SPRINGFIELD, Ill. – Greek American Rehab & Nursing has been fined $10,000 (Statement of Violation), by the Illinois Department of Public Health for failure to provide adequate supervision to prevent a resident from falling down a stairwell. The 204-bed skilled care facility is located at 220 N. First St. in Wheeling.

Responding to a complaint, Department investigators learned a resident sustained multiple traumatic injuries after falling down a stairwell. Despite the resident's history of falling and wandering, surveyors found no evidence that the facility implemented care plans to address these behaviors.

In addition, staff did not respond for at least 15 minutes to an alarm that sounded when the resident opened the stairwell door.

The Department-ordered plan of correction required the facility to provide adequate supervision to residents and to train staff on their responsibilities in responding to door alarms.

Greek American Rehab & Nursing has requested a hearing on the Department's action. No hearing date has been set.
