| December 23, 2003 EAST MOLINE FACILITY FINED FOR INADEQUATE SUPERVISION SPRINGFIELD, Ill. -- The Illinois Department of Public Health has fined Parkview Terrace $5,000 (Statement of Violation) for failure to provide adequate supervision to prevent a resident from leaving the facility unnoticed. The 120-bed skilled and intermediate care facility is located at 430 S. 30th Ave. in East Moline. As part of an annual inspection and complaint investigation, Department investigators learned facility staff was unaware a resident had left the facility until alerted by an employee who saw her walking down the street. The resident was returned to the facility unharmed. Earlier in the day, the resident was seen near the front door and staff was told to monitor the woman every 15 minutes. Despite knowing the resident was attempting to leave, no staff was monitoring the front entrance. It was later determined that the resident left when a salesperson entered the building. A Department-ordered plan of correction required the facility to provide residents with adequate supervision, to train staff on preventing resident elopements and to ensure all resident-accessible exterior doors are equipped with functional alarms or are under visual supervision. Parkview Terrace has requested a hearing on the Department's action. No hearing date has been scheduled. |
