FACILITY FINED FOR NOT SAFEGUARDING RESIDENTS

SPRINGFIELD, IL – The Illinois Department of Public Health has fined Alden Park Strathmoor $10,000 (Statement of Violation), for failure to protect its residents from sexual abuse. The 189-bed skilled care facility is located at 5668 Strathmoor Drive in Rockford.

Responding to a complaint, Department surveyors learned a resident sexually assaulted another resident in a facility shower stall. When staff entered the shower room, they observed a male resident leave and found an undressed female resident in a shower stall.

The woman said she had been raped. The woman was sent to a hospital emergency department about four hours later after two employees had helped her shower.

Prior to the alleged rape, the man had attempted to take two female residents into his room and had been found inappropriately touching another female resident. As a result of these incidents, facility staff had placed him under an informal watch, but they did not re-assess the resident's condition or document the additional monitoring in his plan of care.

Two days after the alleged rape and after another female had been found in his room, the man was transferred to a hospital psychiatric unit.

A Department-ordered plan of correction required the facility to ensure assessments are conducted for all residents involved in an abuse incident, appropriate actions are taken to protect residents from further abuse and interventions are implemented for the individuals committing the abuse.

Alden Park Strathmoor requested a hearing on the Department's action. No hearing date has been scheduled.
