GALESBURG LONG-TERM CARE FACILITY
FINED AFTER RESIDENT INJURED

SPRINGFIELD, Ill. -- The Illinois Department of Public Health has fined Rosewood Care Center Galesburg $5,000 (Statement of Violation) for failure to provide adequate supervision to prevent an accident. The 180-bed skilled care facility is located at 1250 W. Carl Sandburg Drive in Galesburg.

During an incident report investigation, Department surveyors learned a resident, who had a history of wandering, sustained a fractured hip after leaving the facility unsupervised.

When the exit door alarm sounded, an employee instructed staff to check the halls, but did not specify the door that had triggered the alarm as required for in facility policy. After looking out the east patio door and not seeing a resident outside, an employee turned off the alarm and staff did not conduct a head count to determine if a resident was missing. A short time later, a family member reported seeing the resident walking outside and staff found the resident lying on the cement about six feet from a west exit door.

Staff told Department surveyors they were unaware of what action to take after the door alarm sounded.

The Department-ordered plan of correction required the facility to educate staff in their role of providing care to and monitoring of residents to ensure their safety and to ensure that staff is aware of the facility's current policies and procedures regarding missing residents.

Rosewood Care Center Galesburg has requested a hearing on the Department's action. No hearing date has been scheduled.
