FOUR SOUTHWESTERN ILLINOIS FACILITIES DISCIPLINED

SPRINGFIELD, Ill. – The Illinois Department of Public Health has taken disciplinary action against four southwestern Illinois facilities for failure to provide adequate resident supervision:

  • Cardinal Hill Healthcare, a 98-bed skilled and intermediate care facility located at 400 E. Hillview Ave. in Greenville ( Bond County);
  • Eden Village Care Center, a 138-bed skilled care facility located at 400 S. Station Road in Glen Carbon ( Madison County);
  • Pickneyville Health Care Center, a 60-bed intermediate care facility located at 708 Virginia Court in Pickneyville ( Perry County); and
  • Randolph County Care Center, a 120-bed skilled and intermediate care facility located at 312 W. Belmont St. in Sparta ( Randolph County).

Cardinal Hill Healthcare was fined $5,000 (Statement of Violation) for failing to prevent a resident from leaving the facility without staff knowledge. The resident walked across the road to a neighboring farmhouse and was returned to the facility unharmed. The resident was wearing an electronic monitoring device at the time of the incident and it sounded when she was returned to the facility. However, staff did not recall hearing the alarm sound when she left.

Eden Village Care Center was fined $5,000 (Statement of Violation) for an incident in which a resident left the facility unnoticed and was found about one-fourth of a mile away in a residential area. An alarm sounded about the time the resident left the facility, but staff only briefly looked around the area and did not search outside or do a head count of residents to check if anyone was missing.

Pickneyville Health Care Center was fined $5,000 (Statement of Violation) for failing to prevent a resident from leaving the facility undetected. The resident was considered a high risk for leaving the facility and, on the day of the incident, the resident had attempted to leave several times. Staff suspect the resident slipped out a door when a visitor entered. Following the incident, the door was fitted with a lock that can only be opened with a key.

Randolph County Care Center was fined $5,000 (Statement of Violation) after a resident sustained minor scratches when she left the facility without staff knowledge. The resident was found at a nearby gas station by police.

Cardinal Hill Healthcare did not request a hearing on the Department’s action. Eden Village Care Center, Pickneyville Health Care Center and Randolph County Care Center have requested hearings. No hearing dates have been set.
