MOLINE FACILITY FINED FOR INADEQUATE SUPERVISION SPRINGFIELD, Ill. – Rosewood Care Center of Moline has been fined $10,000 (Statement of Violation) by the Illinois Department of Public Health for failure to prevent a resident from leaving the facility unattended. The 120-bed skilled care facility is located at 7300 34 th Ave. in Moline. As part of an incident report and complaint investigation, Department surveyors determined the facility failed to ensure exit doors were alarmed to prevent residents from leaving the facility. The surveyors learned a resident left unnoticed through the facility’s front door, which did not have an alarm, and was not found until more than 10 hours later in a ravine. The resident was treated at a local hospital for hypothermia and dehydration. Rosewood Care Center has requested a hearing on the Department’s action. No hearing date has been scheduled.
