FOUR CHICAGO AREA LONG-TERM CARE FACILITIES FINED SPRINGFIELD, Ill. -- The Illinois Department of Public Health has taken disciplinary action against four Chicago-area long-term care facilities:
Bridgeview was fined $10,000 (Statement of Violation) for failing to provide care to a resident whose blood glucose levels were above normal for five days and who displayed other signs and symptoms of elevated blood sugar levels. Despite some unusual behavior, staff did not assess the changes in the resident's condition to determine a need for medical evaluation and treatment. On the sixth day, the resident was transferred to a local hospital and was diagnosed with a diabetic coma. Golfview was fined $10,000 (Statement of Violation) for not ensuring residents received nursing services to prevent them from getting pressure sores or facilitate healing of the sores. In one instance, the facility failed to discover a resident's pressure sore until it had reached an advanced stage. The facility also did not have complete and consistent documentation of another resident's pressure sores that had not improved. Hearthstone was fined $5,000 (Statement of Violation) for an incident in which a resident sustained a cut forehead after leaving the facility unnoticed. The resident, who had a history of wandering, was found more than 1 mile from the facility. Department surveyors learned the front door of the facility's sheltered care unit did not have an alarm and was to be supervised 24 hours a day by the receptionist. The receptionist, however, admitted leaving her desk a couple of times on the day the resident was discovered missing. Westmont was fined $10,000 (Statement of Violation) following the death of a resident from acute respiratory failure. The resident complained of difficulty breathing and was given oxygen, but when checked more than two hours later, the resident was unresponsive. Staff failed to closely monitor his vital signs and to notify the resident's physician. Bridgeview Health Care Center, Golfview Developmental Center and Westmont Convalescent Center requested hearings on the Department's actions. No hearing dates have been scheduled. Hearthstone Manor did not request a hearing. |
