LOCKPORT DEVELOPMENTALLY DISABLED FACILITY DISCIPLINED

SPRINGFIELD, Ill. - The Illinois Department of Public Health has fined Shady Oaks West $10,000 (Statement of Violation) for failure to provide nursing services in accordance with residents' needs. The 16-bed intermediate care facility for the developmentally disabled is located at 16220 Parker Road in Lockport.

During an annual inspection, Department surveyors observed a resident forcefully hit himself on the head and face repeatedly without any staff intervention or redirection. The resident wore a protective helmet, but was able to remove it. After the resident's face became red and swollen, he was sent to the hospital where he was diagnosed with soft tissue trauma.

The facility also failed to obtain informed written consent for the administration of birth control injections or to ensure injections were based on need. Staff was unsure why the injections were prescribed, but believed it was facility policy that women not on birth control receive the injection.

In addition, Department surveyors found the facility did not ensure residents received therapies described in their individual program plans or provide services to prevent wheelchair dependent residents from developing pressure sores.

A Department-ordered plan of correction required the facility to ensure that adequately trained staff was on duty to meet resident needs and to take measures to protect residents from neglectful situations.

Shady Oaks West has requested a hearing on the Department's action. No hearing date has been scheduled.
