CHILLICOTHE NURSING HOME FINED

SPRINGFIELD, Ill. – The Illinois Department of Public Health has fined Heritage Manor - Chillicothe $10,000 (Statement of Violation) for failure to provide adequate care to meet a resident's nursing and personal needs. The 110-bed skilled care facility is located at 1028 Hillcrest Drive in Chillicothe.

Responding to a complaint, Department surveyors learned a resident fell from his bed five times in four-month period, but the facility did not devise and implement an approach to prevent falls. As the result of the last fall, the resident fractured his neck and died.

The resident's care plan noted a variety of approaches were tried, including the use of side bed rails, placing a call light within the resident's reach, frequently monitoring the resident and having maintenance check bed rails to ensure they were functioning properly.

After the resident's second fall, however, there were no changes in his care plan to reflect additional interventions that may be tried to protect him from injury.

Heritage Manor - Chillicothe has requested a hearing on the Department's action. No hearing date has been set.
