CHICAGO FACILITY FINED FOR INADEQUATE SUPERVISION

SPRINGFIELD, Ill. – The Illinois Department of Public Health has fined South Shore Nursing and Rehab Center $20,000 (Statement of Violation) for failure to provide adequate supervision and monitoring of residents. The 240-bed skilled care center is located at 2649 E. 75 th St. in Chicago.

As part of a complaint and incident report investigation, Department surveyors found the facility failed to adequately monitor three residents identified in need of extra supervision. They included:

  • A resident who set himself on fire while smoking a cigarette and sustained second- and third-degree burns to 30 percent of his body.
  • A wheelchair-bound resident, who was to be supervised when on the toilet, was left unattended and fell. He sustained bruises and lacerations.
  • A resident, who was left unattended in bed with the side rails down, fell out of bed and required sutures to close a cut on her forehead.
South Shore Care Center has requested a hearing on the Department’s action. No hearing date has been set.
