VANDALIA FACILITY FINED FOR INADEQUATE SUPERVISION SPRINGFIELD, Ill. – The Illinois Department of Public Health has fined Cherrywood Health Care Center $5,000 (Statement of Violation) for failure to adequately supervise a resident to prevent him from leaving the facility unnoticed. The 116-bed skilled and intermediate care facility is located at 1500 W. St. Louis Ave. in Vandalia. As part of a complaint and incident report investigation, Department surveyors learned a resident with a history of wandering left the facility without staff knowledge. He was observed not wearing any shoes and walking along a state highway by an off-duty employee and returned uninjured to the facility. Staff did not immediately notify the administrator, the resident’s family or his physician that the resident left the facility without anyone’s knowledge. Once staff made the administrator aware of the incident, the resident’s wife and physician were informed. The administrator said the employee who failed to follow facility policy no longer works at the facility and all other staff have been re-educated on the reporting requirements. The facility also failed to thoroughly assess and update care plans for the resident and four others at risk of leaving the facility unsupervised. Cherrywood Health Care Center did not request a hearing.
