South Shore Nursing and Rehabilitation Center, a 240-bed skilled care facility located at 2649 E. 75 th St. in Chicago, has been fined $5,000 (Statement of Violation) for failure to provide adequate supervision to two residents. A resident, who had a history of wandering, left the facility unnoticed. She was found by paramedics and taken to a local hospital. Another resident, who was blind and not able to get around by himself, was dropped off at an appointment without an escort. The resident never saw the doctor because he remained in the lobby until the driver returned. The facility has not yet requested a hearing, but is still within the allotted time to do so.
