Nursing Home Violations For The Month Of October

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of October. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

Adloff Place, a 16-bed intermediate care facility for the developmentally disabled located at 50 Adloff Lane, Springfield, has been fined $25,000 for failure to implement its abuse and neglect policy for 15 residents. Numerous residents suffered injuries, such as cuts and bruises, and those incidents were not properly processed by staff. Residents also suffered poor hygiene due to neglect. The facility requested a hearing on the Department’s action. After a prehearing, the facility withdrew its hearing request and paid a reduced fine.

Amberwood Nursing and Rehab Center, a 162-bed intermediate and skilled care facility located at 2313 N. Rockton Ave., Rockford, has been fined $60,000 for failure to accurately assess signs of urinary tract infection and respiratory status, which led to the emergency hospitalization of two residents. The facility also failed to have a written hot weather emergency plan. The facility has requested a hearing on the Department’s action. A status hearing is scheduled for April 10.

Chicago Ridge Nursing Center, a 231-bed skilled care facility located at 10602 Southwest Highway, Chicago Ridge, has been fined $55,000 for failure to follow a diet order that resulted in the choking death of a resident. The case was settled with the facility paying a fine.

Independence Place, a 16-bed intermediate care facility for the developmentally disabled located at 1705 S. Park Ave., Herrin, received a notice of license revocation and was fined $10,000 for failure to protect individuals from mistreatment, neglect and abuse. A staff person, who smelled of alcohol while on duty and admitted drinking alcohol prior to coming to work, was allowed to work alone during two shifts, thereby jeopardizing the health and safety of residents. The facility has requested a hearing on the Department’s action. The matter is scheduled for a status conference on March 14.

Orchard Court, a 16-bed intermediate care facility for the developmentally disabled located at 1430 State Route 127 South, Jonesboro, has been fined $15,000 for failure to develop and implement plans of care for residents and failure to have enough staff supervision. As a result, one resident experienced five episodes of non-responsiveness in two months and another fell numerous times from a wheelchair. The facility has requested a hearing on the Department’s action. A status hearing is set for March 9.

Paris Health Care Center, a 128-bed skilled care facility located at 1011 N. Main St., Paris, has been fined $5,000 for failure to supervise a resident identified at risk of leaving the building, who left the building three times without staff’s knowledge. The facility has not requested a hearing on the Department’s action.

Pleasant Hill Village, a 98-bed intermediate care facility located at 1010 W. North St., Girard, has been fined $20,000 for failure to assess, monitor and develop interventions for a resident identified as a risk for abusing other residents. One resident was sent to the hospital after an attack by this resident. The facility requested a hearing on the Department’s action. After a prehearing, the facility withdrew its hearing request and paid a reduced fine.

Sharon Health Care Willows, a 219-bed intermediate care facility located at 3520 N. Rochelle, Peoria, has been fined $37,500 for failure to report and investigate impending sexual assaults, to instruct staff of what constitutes reportable behavior and to assign sex offenders to private rooms. The facility has requested a hearing on the Department’s action. A final hearing is scheduled for April 18 and 19.

Shawnee Christian Nursing Center, a 159-skilled care facility located at 1901 13th St., Herrin, has been fined $31,500 for failure to follow its policies and procedures on abuse, which resulted in allegations of actual and/or potential abuse not being reported or properly investigated. Allegations included instances of mental abuse, pulling a resident’s hair, seclusion of residents, physical abuse and sexual abuse. The facility has requested a hearing on the Department’s action. A status hearing is set for March 16.
