PAXTON NURSING HOME FINED
SPRINGFIELD, IL -- The Illinois Department of Public Health has fined Illinois Knights Templar Home in Paxton $10,000 for failing to monitor a resident who left the facility unnoticed and was later found more than four blocks away sitting in the middle of a road.
The 75-bed skilled and intermediate care facility is located at 450 Fulton St., Paxton, in Ford County.
Responding to a report filed by Illinois Knights Templar Home, Department surveyors learned that a 79-year female resident wandered from the facility early in the morning on Jan. 28. A passerby saw the woman, offered help and called paramedics. She was transported to a local hospital and treated for a fractured wrist, cuts and abrasions.
Staff told surveyors they heard a front door alarm sound around 6:40 a.m., but it was shut off by an unknown person after 10 minutes without anyone checking to see if a resident had left. A bracelet worn by the woman to alert staff of her whereabouts apparently had been removed by the woman. (Statement of Violation)
The facility has requested a hearing on the Department's action. No hearing date has been set.
