H1N1 Influenza Vaccination Pre-registration for Illinois Healthcare Providers

IDPH is coordinating the ordering and delivery of vaccine for the H1N1 influenza vaccination if vaccine is available in early fall.  The pre-registration process identifies you as a provider that agrees to administer vaccine to your patients.  Direct shipment of vaccine will be available for orders of at least 100 doses.
