Correcting Death Records

The fee is $15 for the correction for which one certified copy of the amended record will be issued. Additional copies are $2 if issued at the same time. Payment can be made by check or money order payable to the "Illinois Department of Public Health" (Do not send cash).

Current processing time on mailed requests for ANY CORRECTIONS OR CHANGES to death records that contain the appropriate fee is between 8 to 10 weeks from the date the request is received.

You can indicate the desired changes on the Application for Correction form and send it to:

along with the required $15 fee, which entitles you to one copy of the corrected record. Please make your check or money order payable to the "Illinois Department of Public Health" (Do not send cash). Additional copies of the corrected record are $2 each, when requested at the same time.

Major corrections in the record may require documentary proof to ensure the integrity of the certificate. More than one document may be required in some instances.

If you have trouble downloading the Application for Correction form, you can send a written request to IDPH, Division of Vital Records. Include the decedent's full name, date of death, city and county where the death occurred, if known, and the parent(s) name. Then, indicate what items need to be corrected.
