You can mail the completed application for verification of dissolution of marriage record files, PDF format, or send a letter providing the names of both parties, their dates of birth and the date and place of the dissolution of marriage, if known, and a check or money order (Do not send cash) for the $5 fee made payable to the "Illinois Department of Public Health" to:
Regular mail requests, on average, are processed in eight to 10 weeks. (Please note: this does not include delivery time to and from the office.) Expedited service requests (those requests sent in any way other than regular mail) will be processed within three to five business days of receipt, but will be returned regular mail unless separate payment made payable to UPS ($12) for UPS next day return delivery or a prepaid mailer is enclosed. An adult signature is necessary to accept the UPS delivery. We can UPS packages to your place of work. UPS deliveries cannot be made to post office boxes.

In-person orders can be dropped off for mail out within two business days at the Illinois Department of Public Health, Division of Vital Records office, 925 East Ridgely Ave., Springfield, on Monday through Friday from 10 a.m. to 3 p.m., excluding holidays. PLEASE NOTE: the person requesting the verification will be asked to show a valid photo identification card. |
