NURSING HOME DISCIPLINARY ACTIONS

AUGUST 17, 1999 – Brentwood North Nursing & Rehabilitation Center, 3705 Deerfield Road, Riverwoods. The 248-bed skilled care facility was fined $10,000 for not providing treatment to an 86-year-old resident in a safe manner, which resulted in the woman receiving first and second degree burns. AUGUST 17, 1999 – Little Angels Nursing Home, 1435 Summit St., Elgin. The 50-bed skilled care facility for persons under 22 years of age was fined $10,000 for failing to ensure that clients with tracheotomies were provided the nursing services to meet their needs and for not properly administering medications. AUGUST 11, 1999 – Roosevelt Square, 1501 Shomaker Drive, Murphysboro. The 76-bed intermediate care facility for the developmentally disabled was fined $10,000 for failing to take precautions to prevent a resident from swallowing a cleaning disinfectant and other non-food objects. In addition, during an inspection, Department surveyors found housekeeping carts left unattended, unlocked and accessible to residents. The carts were supposed to be locked, but the locks were broken. (News Release, Statement of Violation)

JULY 22, 1999 – Asta Care Center, 300 W. Lowell, Pontiac. The 85-bed intermediate care facility was fined $10,000 for failing to adequately supervise an 82-year-old resident who wandered unnoticed from the facility. The woman was found lying outside the facility with injuries to her head, shoulder and wrist. (News Release, Statement of Violation)

JULY 22, 1999 – Bel-wood Nursing Home, 6701 Plank Road, Peoria. The 300-bed skilled care facility was fined $10,000 for an incident in which a 92-year-old man left the facility unnoticed when temperatures were minus 5 degrees. The man was spotted traveling down a road in his wheelchair by a staff member coming to work. He was taken to a local hospital and treated for hypothermia and frost bite to his hands. JULY 22, 1999 – Fairview Haven, 605-609 N. Fourth St., Fairbury. The 63-bed skilled care facility was fined $10,000 for failing to properly move a resident, report possible injuries, accurately assess the extent of the injuries or report the injuries to the person’s physician. After being moved by hand rather than the prescribed mechanical lift, the 105-year-old woman was found to have neck and leg fractures. (News Release, Statement of Violation)

JUNE 3, 1999 -- Alden-Long Grove Rehabilitation and Health Care Center, RFD Hicks Road, Long Grove. The 248-bed skilled and intermediate care facility was fined $10,000 for failing to properly supervise a 77-year-old resident with a history of wandering. The woman left the facility unnoticed in January 1999 and was found about two hours later lying on the ground. She was admitted to the hospital in critical condition with complications from hypothermia. (News release, Statement of Violation)

JUNE 3, 1999 -- St. Joseph Home of Chicago, 2650 N. Ridgeway Ave., Chicago. The 173-bed skilled care facility was fined $10,000 for failing to continue cardiopulmonary resuscitation (CPR) on a resident who toppled down a stairwell while strapped into his wheelchair. Facility policy mandates that, once it is begun, CPR should not be stopped unless so instructed by physician or paramedic. (News Release, Statement of Violation)

MAY 21, 1999 -- Cedar Ridge Health Care Center, 1 Perryman St., Lebanon. The 120-bed skilled care facility was fined $5,000 for failing to supervise a resident, who in December 1998, wandered unnoticed from the facility. (News Release, Statement of Violation)

MAY 21, 1999 -- Diamondview, 338 Country Club Road, Centralia. The 16-bed intermediate care facility for the developmentally disabled, was fined $10,000 for failing to ensure residents are not subject to physical or sexual abuse by other residents. Two residents repeatedly assaulted other residents, but the facility did not take steps to address the aggression and protect other residents from harm. (News Release, Statement of Violation)

MAY 21, 1999 -- Westlake Home, 2090 W. Lake Drive, Carlyle. The 160-bed intermediate care facility for the developmentally disabled was fined $10,000 for failing to adequately supervise a resident known to have a history of severe physical aggression. On three occassions in February 1999, the resident attacked other residents and staff. (News Release, Statement of Violation)

MAY 7, 1999 -- Cottonwood Health Care Center, 820 E. Fifth St., Galesburg. The 101-bed intermediate care facility was fined $10,000 for failing to monitor the alcohol intake of a resident or to check him for possible adverse reactions. The 47-year-old man died due to consumed liquid going into his lungs. (News Release, Statement of Violation)

MAY 7, 1999 -- Hart House, 905 N.E. Perry St., Peoria. The 15-bed intermediate care facility for the developmentally disabled was fined $10,000 for failing to protect residents from physical abuse by another resident. In one instance, an abusive resident bit a large part of another resident's ear off during an unprovoked attack. (News Release, Statement of Violation)

APRIL 23, 1999 -- Villas of Shannon, 418 S. Ridge St., Shannon. The 73-bed intermediate care facility was fined $10,000 for failing to properly supervise a resident who twice wandered unnoticed from the facility this winter and on one occasion was found walking in the snow without shoes. The woman required treatment to her feet for large blisters and swelling. (News Release, Statement of Violation)

APRIL 20, 1999 -- Illinois Knights Templar Home, 450 Fulton St., Paxton. The 75-bed skilled and intermediate care facility was fined $10,000 for failure to properly monitor a resident. A 79-year-old woman left the facility unnoticed and was found more than four blocks away sitting in the middle of a road. She suffered a fractured wrist, cuts and abrasions. APRIL 7, 1999 -- Alden-Long Grove Rehabilitation and Health Care Center, RFD Hicks Road, Long Grove. The 248-bed skilled and intermediate care facility was fined $10,000 for failing to provide necessary supervision to two elderly women residents who wandered unnoticed from the facility. (News release, Statement of Violation)

APRIL 7, 1999 -- Mother Theresa Home, 1270 Franciscan Drive, Lemont. The 150-bed skilled, intermediate and sheltered care facility was fined $10,000 for not beginning cardiopulmonary resuscitation or calling 911 when a resident was found slumped in a wheelchair and unresponsive. The resident did not have a "Do Not Resuscitate" order, which meant that all appropriate life-saving measures should have been attempted. (News release, Statement of Violation)

MAR. 18, 1999 -- Carlinville Estates, 1221 S. Plum St., Carlinville. The 16-bed intermediate care facility for the developmentally disabled was fined $10,000 for failure to protect residents from repeated incidents of physical, verbal, sexual and psychological abuse by another resident. JAN. 20, 1999 -- Meadow Manor, 800 McAdam Drive, Taylorville. The 150-bed intermediate care facility was fined $10,000 for failure to properly monitor a 78-year-old resident with Alzheimer's disease who left the building undetected and was found at a nearby YMCA. JAN. 20, 1999 -- Swann Special Care Center, 109 Kenwood Road, Champaign. The 104-bed long-term care facility for persons under 22 years of age was fined $10,000 for failure to provide the necessary nursing care to meet the needs of its residents. A 21-month-old resident died of respiratory arrest after being left alone with her breathing monitor switched off. (News release, Statement of Violation)

JAN. 4, 1999 – Patterson House, 307 E. Jefferson, Sullivan. The 16-bed intermediate care facility for the developmentally disabled was fined $20,000 for failure to implement a policy regarding abuse and neglect and for not meeting the nursing care needs of residents. The facility recorded 25 instances of physical abuse by a resident of staff and other residents, but steps were not taken to protect those at the facility from harm. DEC. 8, 1998 -- Gateway Terrace, Route 177, Irvington. The 67-bed intermediate care facility for the developmentally disabled was fined $30,000 and the Department moved to revoke the facility's license for repeated failure to ensure residents were not subjected to physical or psychological abuse and for not investigating allegations of injuries and neglect.

DEC. 8, 1998 -- Westside Care Center, 601 N. Columbia, West Frankfort. The 96-bed skilled care facility was fined $10,000 for failure to supervise residents to prevent elopement and for not ensuring exit door alarms operate effectively.

NOV. 17, 1998 -- Convalescent Care Center, 1000 Palm, Mattoon. The 178-bed skilled care center was fined $10,000 for not adequately supervising two residents who wandered unnoticed from the facility. (News release, Statement of Violation)

OCT. 9, 1998 -- Glenshire Nursing and Rehabilitation Centre, 22660 S. Cicero Ave., Richton Park. This 194-bed intermediate and skilled care facility was fined $10,000 for failure to have in place policies and procedures to be followed during a medical emergency and for not meeting the nursing care needs of its residents. SEPT. 16, 1998 -- Woodside Extended Care, 120 W. 26th St., South Chicago Heights. The 112-bed intermediate and skilled care facility was fined $10,000 for failure to have emergency oxygen available for a resident who experienced respiratory arrest and subsequently died. (Statement of Violation)

SEPT. 16, 1998 -- Maplewood Care, 50 N. Jane, Elgin. This 203-bed skilled care facility was fined $10,000 for failing to implement a plan of care that would prevent a resident from inflicting injuries on himself. (Statement of Violation)

JULY 31, 1998 -- Rosewood Care Center of East Peoria, 900 Centennial Drive, East Peoria. The 120-bed skilled care facility was fined $10,000 for inadequate staffing, which led to neglect of its residents and failure to provide residents with needed care and services, including proper care of pressure sores. (Statement of Violation)

JULY 31, 1998 -- Fondulac Woods Health Care Center, 901 Illini Drive, East Peoria. This 98-bed skilled care facility was fined $10,000 for failing to provide adequate and properly supervised nursing care for a resident who needed tracheostomy care. (Statement of Violation)

JULY 23, 1998 -- Virgil Calvert Nursing and Rehabilitation Center, 5050 Summit Ave., East St. Louis. The 150-bed skilled care facility was fined $10,000 for failing to provide adequate and properly supervised nursing care to residents, including failure to administer life-sustaining treatment to an unresponsive resident and failure to investigate possible abuse of a resident. JUNE 29, 1998 -- Riverbluff Nursing Home, 4401 N. Main St., Rockford. This 304-bed skilled care facility was fined $10,000 for failure to accurately assess the physical condition and to give CPR to a resident with full code status, failure to follow its policy on advance directives and failure to have an effective system in place by which to identify the code status of each resident at all times. JUNE 17, 1998 -- Maplewood Health Care Center, 310 Banbury Road, North Aurora. The 129-bed skilled facility was fined $10,000 for failing to equip all exterior doors with a signal that would alert staff of a resident leaving the building. A resident exited to a courtyard without staff knowledge and was later found to have died due to acute congestive heart failure due to hypothermia. (Statement of Violation)

JUNE 9, 1998 - Gateway Terrace, Route 177 West, Irvington. This 67-bed intermediate care facility for the developmentally disabled was fined $10,000 for failing to provide ongoing assessment of a resident's physical therapy needs, his neurological status and his tolerance to medication changes. (Statement of Violation)

JUNE 4, 1998 - Elmwood Nursing and Rehabilitation Center, Route 159 and Interstate 55/70, Maryville. The 104-bed intermediate and skilled care facility was fined $10,000 for failing to notify a resident's physician of significant changes in her condition, failing to insure that all orders concerning the resident were directed by her physician, failing to follow physician orders, failing to provide liquids to sustain life and failing to administer medications as ordered by the resident's physician.
