Hospital-based Quality Improvement Initiatives in Illinois


Illinois CAPTURE Stroke Registry
The Department is establishing and implementing a Paul Coverdell National Acute Stroke Registry in Illinois called the Illinois CAPTURE Stroke Registry. Illinois is one of four states (others being Georgia, Massachusetts and North Carolina) to receive registry funding. The data collected will be used to measure quality of care for acute stroke patients and to implement improvement strategies that focus on policy, systems change and professional education to improve treatment. The Department, in collaboration with the University of Illinois College of Medicine at Chicago, will implement the registry. A total of 24 hospitals agreed to participate in 2005 data collection. At the end of the third year (June 2007), approximately 100 hospitals will be part of the Illinois CAPTURE Stroke Registry. The initial training for hospital staff was held in February 2005.  (Stroke Registry $892,900, first year of three-year project

 

Get With the Guidelines (GWTG)
GWTG is an American Heart Association/American Stroke Association (AHA/ASA), full-scale, ready-to-implement continuous quality improvement program designed to ultimately save lives by quickly and positively impacting in-hospital patient care through the consistent application of the most up-to-date scientific, evidence-based guidelines, thereby, reducing patient risk factors and the incidence of recurring, catastrophic events related to cardiovascular diseases and stroke. GWTG utilizes an online Patient Management Tool™ , an interactive reporting and assessment system. This user-friendly, customizable quality improvement tool assists hospitals in reaching their GWTG goals by putting the most up-to-date information at their fingertips; by integrating timesaving features such as guideline checks on treatments and automatically generated, patient-specific education materials; and by providing real-time decision support.

 

GWTG-Coronary Artery Disease
GWTG-CAD is an AHA continuous quality improvement program designed to save lives by quickly and positively impacting in-hospital patient care through the consistent application of the most up-to-date scientific guidelines on coronary artery disease, thereby reducing the incidence of recurring, catastrophic events related to coronary artery disease as well as providing primary prevention for risk of stroke.

 

GWTG-Stroke
GWTG-Stroke is an ASA continuous quality improvement program designed to reduce risk factors and save lives by quickly and positively impacting in-hospital patient care through the consistent application of the most up-to-date scientific guidelines on stroke, thereby reducing the incidence of recurring, catastrophic events related to stroke.

 

GWTG-Heart Failure
GWTG-Heart Failure is an AHA continuous quality improvement program designed to save lives by quickly and positively impacting in-hospital patient care through the consistent application of the most up-to-date scientific guidelines on heart failure, thereby reducing the incidence of recurring, catastrophic events related to heart failure.
