NURSING HOME VIOLATIONS FOR THE MONTH OF SEPTEMBER

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of September. An “A” violation, which is the most serious licensure violation levied by the state, creates a condition in which there is a substantial probability that death or serious mental of physical harm will result.

The statement of violation for each facility can be obtained by visiting the Department’s Web site at www.idph.state.il.us.

All American Nursing Home, a 144-bed skilled and intermediate care facility located at 5448 N. Broadway St. in Chicago, has been fined $10,000 (Statement of Violation) for failure to provide a resident with adequate supervision and assistance to prevent accidents. After leaving the facility unnoticed, a resident threw himself in front of a commuter train and was killed. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Care Centre of Urbana, a 99-bed skilled care facility located at 907 N. Lincoln Ave. in Urbana, has been fined $5,000 (Statement of Violation) for failure to monitor the whereabouts of a resident with wandering tendencies and to account for all residents prior to resetting the door alarm. The resident was able to leave the facility without staff knowledge. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Coventry Village, a 130-bed skilled and sheltered care facility located at 612 W. St. Mary’s St. in Sterling, has been fined $20,000 (Statement of Violation) for failure to have a system to correctly identify residents prior to the administration of medications, to respond appropriately to hypoglycemic episodes, and to follow physician’s orders when resident’s blood glucose levels fell below the established parameters. These failures contributed to a resident having four significant episodes of hypoglycemia, including one that resulted in hospitalization. The fine also is the result of the facility’s failure to recognize a heat emergency, to implement procedures to ensure the well being of residents and to have a detailed written heat emergency plan. The facility’s cooling system was not functioning and inside temperatures reached more than 80 degrees Fahrenheit. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Douglas Rehab & Care Center, a 79-bed skilled care facility located at 3516 Powell Lane in Mattoon, has been fined $10,000 (Statement of Violation) for failure to initiate emergency procedures for a non-responsive resident and to assure that 17 certified nursing assistants and three nurses on staff were current in cardiopulmonary resuscitation certification. The facility has requested a hearing. No hearing date has been scheduled.

Gardens – La Grange Victorian Manor, a 203-bed skilled and intermediate care facility located at 339 S. 9 th Ave. in La Grange, has been fined $5,000 (Statement of Violation) for failure to provide adequate supervision to prevent a resident from leaving the facility unnoticed. The resident was not found until the following morning. Staff did not perform hourly checks as required by the facility’s elopement policy. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Hawthorne Inn of Clinton, a 98-bed skilled care facility located at 1 Park Lane West in Clinton, has been fined $10,000 (Statement of Violation) for failure to promptly follow a physician’s order to obtain a Computerized Tomography (CT) scan of a resident, to assess a change in a resident’s condition and to notify the oncoming nursing staff and physician of this change in condition. These failures caused a delay in medical treatment for a resident with abdominal pain who died before emergency services could be called. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Helia Healthcare of Carbondale, a 118-bed skilled care facility located at 500 Lewis Lane in Carbondale, has been fined $5,000 (Statement of Violation) for failure to provide adequate supervision to prevent a resident from leaving the facility unnoticed. The resident was found five miles away and taken to a hospital emergency department suffering from dehydration. The facility’s investigation found an exit door alarm was low pitched and could not be heard by staff. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Medina Nursing Center, an 89-bed skilled care facility located at 402 S. Center St. in Durand, has been fined $5,000 (Statement of Violation) for failure to prevent a resident from leaving the facility unsupervised. After hearing a door alarm sound, an employee looked outside before turning off the alarm. A resident wearing an electronic monitoring device had gotten outside and was standing in the parking lot at the time. The employee thought the resident was a visitor. The resident was found about 15 minutes later walking down the road about half a mile from the facility. The facility has requested a hearing. No hearing date has been scheduled.

Pekin Living & Rehab Center, a 202-bed skilled care facility located at 2220 State St. in Pekin, has been fined $55,000 (Statement of Violation) for failure to prevent abuse or neglect of a resident and to properly assess and monitor the condition of another resident who was insulin dependent.

In the case of the first resident, an employee removed the resident’s oxygen and, with the help of three other staff members, injected him with a large dose of an anti-psychotic drug that was not ordered by a physician. Later that evening, the resident sustained a serious head injury in a fall and died. In another incident, the facility failed to assess and monitor the condition of a diabetic resident, who was not receiving prescribed insulin injections. The facility has not yet requested a hearing, but is still within the allotted time to do so.
