NURSING HOME VIOLATIONS FOR THE MONTH OF FEBRUARY

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act for the month of February 2005.

The statement of violation for each facility can be obtained by visiting the Department’s Web site at www.idph.state.il.us and clicking on the phrase “statement of violation” contained in the paragraph about the facility.

Alden Princeton Rehabilitation and Health Care Center , a 225-bed skilled care facility located at 255 W. 69 th St. in Chicago, has been fined $20,000 (Statement of Violation) for failure to ensure that a resident received the necessary care and treatment for multiple pressure sores on her right leg and right heel. The resident developed numerous layers of maggots in these wounds and was later hospitalized with a severe infection. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Cedar Ridge Healthcare Center , a 114-bed skilled care facility located at One Perryman St. in Lebanon, has been fined $10,000 (Statement of Violation) for failure to prevent the potential sexual abuse of two residents by another resident, who had a history of sexually inappropriate behaviors. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Courtyard Terrace Nursing Home , a 162-bed skilled and intermediate care facility located at 2313 N. Rockton Ave. in Rockford, has been fined $10,000 (Statement of Violation) for failure to provide adequate supervision to prevent residents on the Alzheimer’s disease unit from having accidents. A total of eight residents had injuries of unknown origin or fell without staff supervision. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Embassy Care Center , a 171-bed skilled and intermediate care facility located at 555 W. Kahler Road in Wilmington, has been fined $5,000 (Statement of Violation) for failure to provide adequate supervision to prevent a resident from leaving the facility unnoticed. The resident was returned to the facility uninjured. The facility has requested a hearing. No hearing date has been scheduled.

Glen Oaks Nursing & Rehabilitation Center , a 298-bed skilled and intermediate facility located at 270 Skokie Highway in Northbrook, has been fined $10,000 (Statement of Violation) for failure to prevent a resident from leaving the facility unnoticed. The resident was found across the street from the center and was transported to a hospital, where he was treated for hypothermia and then returned to the facility. The facility has requested a hearing. No hearing date has been set.

Heartland Health Care Center – Paxton , a 96-bed skilled care facility located at 1001 E. Pells St. in Paxton, has been fined $10,000 (Statement of Violation) for failure to properly supervise a resident, who fell off a toilet. The resident underwent surgery to stop bleeding under the scalp, but later died. The facility has requested a hearing. No hearing date has been scheduled.

Rosewood Care Center of Elgin , a 139-bed skilled care facility located at 2355 Royal Blvd. in Elgin, has been fined $5,000 (Statement of Violation) for failure to supervise an exit door when the alarm is turned off. A resident was able to leave the facility undetected after a maintenance man turned off a door alarm. The facility has requested a hearing. No hearing date has been scheduled.

Stephenson Nursing Center , a 162-bed skilled and intermediate care facility located at 2946 S. Walnut Road in Freeport, has been fined $5,000 (Statement of Violation) for failure to provide adequate supervision to prevent a resident from leaving the facility unnoticed and to ensure that exit door alarms sound when doors are opened. The facility has requested a hearing. No hearing date has been scheduled.
