NURSING HOME VIOLATIONS FOR THE MONTH OF AUGUST

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of August. An “A” violation, which is the most serious licensure violation levied by the state, creates a condition in which there is a substantial probability that death or serious mental of physical harm will result.

The statement of violation for each facility can be obtained by visiting the Department’s Web site at www.idph.state.il.us.

Asta Care Center of Rockford, a 130-bed skilled and intermediate care facility located at 707 W. Riverside Blvd. in Rockford, has been fined $16,500 (Statement of Violation) for failure to provide adequate nursing care. The facility did not check a diabetic resident’s blood sugar level before giving him insulin. The resident had a hypoglycemic reaction and went into cardiac arrest. Twelve other diabetic residents had problems with the management of their diabetes related to missing blood sugar monitoring, wrong doses of insulin and insulin not given as ordered or not given at all. The facility has requested a hearing. No hearing date has been set.

Cedarwood Health Care Center, a 58-bed intermediate care facility located at 136 South Dipper Lane in Decatur, has been fined $5,000 (Statement of Violation) for failure to check the Nurse Aide Registry prior to allowing a new employee to provide direct resident care. This resulted in a certified nursing assistant, who had a finding of abuse on the Nurse Aide Registry and a disqualifying crime, being verbally abusive to a resident. After the facility’s investigation, the employee was fired. The facility has requested a hearing. No hearing date has been set.

Colonial Apartments, a 16-bed intermediate care facility for the developmentally disabled located at 920 W. Fourth St. in Centralia, has been fined $5,000 (Statement of Violation) for failure to provide one-on-one supervision to a resident in order to prevent him from leaving the facility unsupervised, and to address his elopement and ingestion of inedible objects in a formal training program. The resident, who often sought out potentially dangerous food sources, was found walking toward a dumpster. The facility has requested a hearing. No hearing date has been set.

Colonial Manor, a 49-bed intermediate care facility for the developmentally disabled located at 300 Church St. in Zeigler, has been fined $5,000 (Statement of Violation) (Statement of Violation) for failure to have in place protective measures before allowing residents on a bus with a driver who slapped a resident. In addition, the facility failed to investigate an incident in which a resident left the facility and to take corrective action to prevent other attempts to leave the facility. As a result, the resident was able to leave the facility again without staff knowledge. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Danville Care Center, a 200-bed skilled and intermediate care center located at 1701 N. Bowman Ave. in Danville, has been fined $5,000 (Statement of Violation) for failure to monitor the whereabouts of a resident known to wander and to ensure two exit doors were supervised when the door alarms were deactivated. A resident left the facility without staff knowledge and was found by police near a viaduct and railroad tracks. Following the incident, the resident was fitted with an electronic monitoring device and 15-minute checks were initiated. The facility has requested a hearing. No hearing date has been set.

East Peoria Gardens Healthcare Center, a 103-bed skilled and intermediate care facility located at 1910 Springfield Road in East Peoria, has been fined $20,000 (Statement of Violation) for failure to perform appropriate emergency response measures for a resident who was found gasping for air. Staff did not attempt rescue procedures and were not trained on the correct ways to assist a choking victim. The resident was found dead in his wheelchair by paramedics responding to the call. In addition, the facility failed to adequately supervise and monitor another resident, who was on a pureed diet, during mealtime. The resident choked on a peach slice and died several days later at a local hospital. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Mercy Health Care Rehab Center, a 259-bed skilled care facility located at 19000 Halsted Ave. in Homewood, has been fined $5,000 (Statement of Violation) for failure to prevent a resident from leaving the facility unsupervised. The resident was able to exit the facility through the dock doors, which were not locked following a delivery. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Oregon Healthcare Center, a 104-bed skilled-care facility located at 811 S. 10 th St. in Oregon, has been fined $10,000 (Statement of Violation) for failure to check the Nurse Aide Registry prior to hiring an employee, to immediately report to the administrator and investigate an allegation of abuse of a resident by this staff member and to remove the employee from contact with residents pending an investigation of the allegation. The facility has requested a hearing. No hearing date has been set.

Pekin Living and Rehab Center, a 202-bed skilled care facility located at 2220 State St. in Pekin, has been fined $5,000 (Statement of Violation) for failure to prevent a resident from leaving the facility unsupervised. Staff failed to do 15-minute checks on the resident as required by his care plan. The resident was found walking approximately one mile from the facility. Pekin Living and Rehab Center has requested a hearing. No hearing date has been set.

Provena St. Joseph Center, 120-bed skilled care facility located at 659 E. Jefferson St. in Freeport, has been fined $30,000 (Statement of Violation) for failure to prevent three female residents from being sexually abused by a male resident. Staff did not know how to respond to and relay three witnessed incidents of sexual abuse to administration and did not follow the male resident’s care plan and recommendations of his psychologist. These failures contributed to a fourth incident of sexual abuse. The facility has requested a hearing. No hearing date has been set.

Redwood Manor, a 58-bed intermediate care facility located at West Franklin St. in Sesser, has been fined $10,000 (Statement of Violation) for failure to notify the administrator that an allegation of abuse had been filed against him, to arrange for the administrator’s suspension so that an investigation could be conducted, to thoroughly investigate the alleged abuse and to follow facility policy for investigating the potential abuse of a resident. The facility has not yet requested a hearing, but is still within the allotted time to do so.

Three Springs Lodge Nursing Home, an 83-bed skilled and intermediate facility located at 161 Three Springs Road in Chester, has been fined $5,000 (Statement of Violation) for failure to prevent a resident from leaving the facility unnoticed. A resident, who had a history of attempting to leave the facility, was seen walking down the middle of the street by a motorist. The resident fell and sustained a bloody nose. The door alarms in the facility’s new addition were not working at the time. The facility had a new alarm system installed following the incident. The facility did not request a hearing and has paid the fine.
