Nursing Home Violations For The Month Of April

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of April. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

Alpine Fireside Health Center, a 99-bed intermediate and sheltered care facility located at 3650 N. Alpine Rd., Rockford, has been fined $20,000 for failure to identify, assess and seek treatment for a resident with a fracture of the leg. These failures resulted in delayed care and prolonged pain for the resident. The facility has requested a hearing on the Department’s action. The matter is set for status on December 10.

Cardinal Hill Health Care, a 90-bed s killed care facility located at 400 E. Hillview, Greenville, has been fined $22,500 for failure to adequately monitor, access and immediately consult with a physician about a change in the resident’s condition. This failure resulted in the resident not receiving timely treatment. The resident died hours after the first evidence of decline of condition. The facility has not requested a hearing on the Department’s action.

Colonial Plaza, a 16-bed intermediate care facility for the developmentally disabled located at 618 W. Goodner, Nashville,has been fined $11,000 for failure to implement its policies and procedures to prohibit mistreatment, abuse and neglect when a resident naked from the waist down was guided through common areas of the facility. The facility also failed to have a responsible direct care staffer awake during the midnight shift to care for the residents. The facility has requested a hearing on the Department’s action. The matter is set for status on November 29.

Fairview Nursing Plaza, a 213-bed intermediate and skilled care facility located at 321Arnold Ave., Rockford, had been cited for failure to ensure bed rails and side rails were free from accident hazards. A resident was found dead by staff after reportedly becoming wedged between the bed frame and side rail. The facility requested a hearing on the Department’s action. The final order was signed and the facility paid a fine of $11,525.

Friendship House of Centralia, a 94-bed intermediate and skilled care facility located at 1000 Martin Luther King, Centralia, had been cited for failure to provide properly applied bed bolsters for a resident assessed as needing to use bed bolsters for positioning. This failure resulted in the resident falling from the bed and sustaining head injury. The facility has requested a hearing on the Department’s action. The final order was signed and the facility paid a fine of $15,000.

Helia Healthcare of Champaign, a 118-bed skilled care facility located at 1915 S. Mattis St., Champaign, has been fined $50,000 for failure to safely transfer a resident into a shower stall. A resident tipped over in a shower chair, head hitting the shower wall. The resident suffered a neck fracture and died. The facility has not requested a hearing on the Department’s action.

Lakeview Living Center, a 145-bed intermediate care facility for the developmentally disabled located at 7270 S. Shore Dr., Chicago, has been fined $11,000 for failure to implement written policies and procedures for an incident involving an allegation of possible neglect and abuse. Staff did not closely monitor a resident diagnosed for inappropriately touching others. This resident was found in bed with another resident. The facility has requested a hearing on the Department’s action. The hearing is set for February 5, 2008.

Oakview, a 16-bed intermediate care facility for the developmentally disabled located at 2311 Veterans Dr., Effingham, has been fined 15,000 for failure to implement its policies to prevent abuse and neglect. Staff failed to prevent and investigate verbal and physical abuse of residents by one resident. The facility has requested a hearing on the Department’s action. A status in the matter is set for December 13.

Orchard Court, a 16-bed intermediate care facility for the developmentally disabled located at 1430 State Route 127 South, Jonesboro, has been fined $5,000 for failure to implement its own policy and procedures for supervision and special observation of a resident who left the facility without staff knowledge. Staff did not realize the resident had left until the resident later returned. The facility has requested a hearing on the Department’s action. A status in the matter is set for November 29.

Pavillion of Forest Park, a 232-bed skilled care facility located at 8200 W. Roosevelt Rd., Forest Park, has been fined $35,000 for failure to identify, treat, provide appropriate preventive measures, assess and care plan residents at risk for pressure ulcers. This failure was systematic affecting at least 13 residents sampled. The facility has requested a hearing on the Department’s action. A status in the matter is set for January 3.

Pleasant Hill Village, a 98-bed intermediate care facility located at 1010 W. North St., Girard, has been fined $25,000 for failure to ensure that residents were not physically verbally or mentally abused by staff. The facility also failed to report allegations, thoroughly investigate and take steps to prevent further abuse while investigations were in progress. The facility has requested a hearing on the Department’s action. A status in the matter is set for November 29.

Robings Manor Rehab & Health Care, a 75-bed intermediate and skilled care facility located at 502 N. Main, Brighton, had been cited for failure to ensure that residents received treatment to prevent the development of pressure sores. This resulted in harm to two residents who developed pressure sores, including a resident who developed sepsis requiring surgery of the wound. The facility has requested a hearing on the Department’s action. The final order was signed and the facility paid a fine of $13,000.

Rosewood Care Center St. Charles, a 109-bed skilled care facility located at 850 Dunham Rd., St. Charles, has been fined $10,000 for failure to thoroughly investigate the circumstances related to incidents involving two residents with injuries of unknown origin with resulted in untreated pain and possible fracture. The facility has requested a hearing on the Department’s action. A status in the matter is set for January 10.

Seguin RCA Harvey House, a 16-bed intermediate care facility for the developmentally disabled located 3309 S. Harvey Ave., Berwyn, has been fined $15,000 for failure to implement policies and procedures to prevent neglect of a resident. The resident fell and hurt an arm, but was not taken to the hospital until 6 days later when x-rays confirmed a fracture. The facility has requested a hearing on the Department’s action. A status in the matter is set for January 14.

Springwood Nursing and Rehab, a 97-bed intermediate and skilled care facility located at 1920 N. Main St., Rockford, has been fined $5,000 for failure to supervise and ensure the safety of two residents who left the second floor on a secured elevator. The main floor alarm was not programmed to alert staff when the door opened which resulted in the residents leaving the facility without staff knowledge. The residents who were not properly dressed were found outside in 19 degree weather. The facility has requested a hearing on the Department’s action. A status in the matter is set for November 29.

Westshire Nursing & Rehab Center, a 485-bed intermediate and skilled care facility located at 5825 W. Cermak Rd., Cicero, has been fined $15,000 for failure to ensure the safety of residents by not adequately supervising one resident after accusations of sexual assault against another resident. This failure resulted in other residents being put in potential harm after the allegation of sexual assault was made known. The facility has requested a hearing on the Department’s action.
